If you did not receive a claim form and you failed the LAST, you may download a copy by clicking on the link below. Please note that there are two different claim forms, one for individuals who failed the LAST for the first time before February 14, 2004 and another for those who failed the LAST for the first time on or after February 14, 2004. You can complete the claim form and submit it online via the Submit Claim Form Online link below. In order to submit your Social Security Administration Release form, please complete the form and send to Garden City Group at Gulino v.
